If the insured person changes his or her name due to marriage or divorce, certain procedures must be followed.

Procedures for a Change of Name

When you change your name Please notify your company immediately, attach a Health Insurance Eligibility Certificate (only if issued) to the application form and submit it.
For those who have a Health Insurance Eligibility Certificate, the health insurance society will issue new Health Insurance Eligibility Certificate with your new name based on information from your employer.
* Even if you use your maiden name in-house, you must notify the health insurance association.
* There is no need to attach your Individual Number Card.
