被保険者の皆様

ZARA健康保険組合では、皆様の歯の健康をサポートするため、

令和8年3月より歯科クリーニングへの補助金支払いを開始いたします。

対象者は被保険者（本人）のみで、被扶養者（ZARA健康保険組合に加入している配偶者・子ども等）は除きます。

実施期間は、令和8年3月1日から令和8年5月31日までの3か月間です。

健康保険対象外の自費によるクリーニング（ホワイトニングを含む）を対象とします。

補助金の上限は5,000円（税込み）で、上限を超えた部分は自己負担となります。申請は1回のみとなります。

申請書は「各種申請書」に用意しましたので、必要事項を記載の上、領収書とともにZARA健康保険組合へ郵送してください。

費用は、令和8年4月以降に給料に含めて支払います。任意継続被保険者の方は登録口座に振込となります。

 Notice of the Start of Dental Cleaning Subsidy Payments

To all insured members,

The ZARA Health Insurance Association will start providing subsidies for dental cleanings from March 2026 to support your dental health.

The subsidy is available only to insured members (the individual policyholders) and does not cover dependents (spouses, children, etc., enrolled in ZARA Health Insurance).

The implementation period is three months, from March 1, 2026, to May 31, 2026.

The subsidy applies to dental cleanings paid out-of-pocket and not covered by health insurance.

The maximum subsidy is 5,000 yen (including tax); any amount exceeding this limit will be paid by the individual. Applications can be made only once.

Please fill in the required information on the reimbursement form, which will be provided later, and send it to the ZARA Health Insurance Association along with the receipt.

The cost will be included in your salary payment from April 2026 onwards. For voluntary continued insured members, payment will be made via bank transfer to the registered account.

If you have completed dental cleaning before the reimbursement form is ready, please keep your receipt.
